What are the issues that you have faced due to Non-Compliance of procurement policies?

I understand that most corporations have policies for procurement for instance: all purchases should be made through a definitive procedure, PO should be raised using tool like Ariba, local procurement team should not accept gifts from suppliers etc. What are the issue that you have faced due to non-compliance of such policies?
