on-line questionnaires for qualifying suppliers

Looking to gain insight into what people are using to build on-line assessments and questionnaires? These are then shared with suppliers and partners to help in the qualification and management of those companies. Has anyone experience with current SaaS providers in this area? I am particularly interested in the pharmaceutical and medical device sectors.
